PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Social prescribing is an all-age, whole population approach that works particularly well for people who:

Clinical Pharmacist
Clinical pharmacists are increasingly working as part of general practice teams. They are highly qualified experts in medicines and can help people in a range of ways. Podiatrist
