To register at The Upstairs Surgery, you must follow the below steps you can visit the practice in-person to collect the forms on Mondays or Tuesdays between 14:00pm - 17:30pm

Patients can only return these on a Thursday 09:30am - 12:30pm - if you are unable to attend surgery to return these, you can email these on the allocated day.
Alternatively, you can print the forms at the comfort of your own home, then drop them off at the practice.

Below, are the requirements you need to register.
Step One: Check if you meet the criteria

Live within the practice area

Provide proof of identification e.g. driving licence, passport, birth certificate, current visa or identity card.

Provide proof of address e.g. utility bill, council tax bill, bank statement (dated within the last 3 months).

Mobile phone invoices will not be accepted.

Patients under the age of 7 years will need to provide a birth certificate and immunisation history (red book).
Step Two: Sign forms

Please complete ALL the forms. This includes the registration form and the New Patient Questionnaire. You can collect them at the reception or print them out yourselves. Failure to do this will result in an unsuccessful application.​

If you need help with language interpretation, please visit at our GP reception
Step Three: Give forms and documents to receptionists

Once completed, please hand to the receptionist on the date previously advised. The surgery will handle your application. If your application is unsuccessful you will be contacted and your paperwork will be left at reception for collection.
